I hearby grant Lake Forest Park Shoreline Lacrosse Club (Shoreline Lacrosse) and its agents permission to take and use photographs and/or video and/or audio recordings of me and/or my child(ren). Shoreline Lacrosse retains the right to use those photographs, videos, and/or audio recordings in any and all of its publications, including but not limited to flyers, advertisements, web-based publications, and social media feeds, without payment or other consideration. Parents retain the right to request that pictures, video and/or audio be removed from public display at any time.
