Liability and Media Waiver

WAIVER AND RELEASE

In consideration of my participation in a Shoreline LFP Lacrosse interaction, I am fully aware of and accept the risks, including the risk of catastrophic injury, paralysis, and even death, as well as other damages and losses, associated with participation in a lacrosse-related event. I further agree, on behalf of myself, my heirs, and personal representatives, that Shoreline LFP Lacrosse shall not be liable for any injury, loss of life, or other loss or damage occurring as a result of my participation in any such Shoreline LFP Lacrosse recognized or sanctioned event.



MEDIA RELEASE

In consideration of my Shoreline LFP Lacrosse interaction, I, grant Shoreline LFP Lacrosse and its volunteers, representatives, directors, and officers (collectively, “Shoreline LFP Lacrosse”) the license to use my name image, photograph, voice, video, athletic performance, biographical, and other information (collectively, “Likeness”), in any media platform or format whatsoever, and to distribute, broadcast and exhibit these without charge, restriction or liability and such shall be the property of Shoreline LFP Lacrosse in perpetuity. I hereby release and discharge Shoreline LFP Lacrosse from any and all claims, demands, or causes of action arising out of or in connection with the use of my likeness for the purposes described herein.
